Benefits
Medical Benefits
Ohio Health offers three plans to choose from.
Dental Benefits
OhioHealth offers dental coverage through Delta Dental. The Plan
has three tiers: Preferred, Premier, and Out of Network. Your
reimbursement is determined by the provider you choose.
Vision Service Plan (VSP)
VSP is a group vision plan that operates with a provider network.
VSP promotes wellness through a comprehensive eyecare program.
The plan provides for periodic eye examinations, once every 12
months, as well as other services and corrective eyewear needed
to correct the patient's visual needs.
Flexible Spending Accounts
OhioHealth offers Health Care and Dependent Care Spending Accounts
on a pre-tax basis, which provides you with tax savings. Besides
saving on your taxes, these Spending Accounts let you budget
and allocate your out-of-pocket health and dependent care expenses
more evenly throughout the year.
Health Care Spending Account
In this account, you can set aside up to $4,000 a year ($130
minimum) in pre-tax money to pay for your out-of-pocket medical,
dental, vision and hearing care expenses.
Dependent Care Spending Account
This account allows you to pay daycare expenses up to $5,000
($130 minimum) for children and disabled or elderly family members
with before-tax dollars.
Basic Life Insurance for You
OhioHealth offers group term life insurance at several levels
to help meet your need for financial protection. You can choose
from the following options for life insurance on yourself:
- Flat amount of $10,000
- 1 times your annual salary (up to $150,000)
- 2 times your annual salary (up to $300,000)
- 3 times your annual salary (up to $450,000)
- 4 times your annual salary (up to $600,000)
The cost for this insurance protection depends on the amount
of the insurance you select. Evidence of insurability is required
for coverage over $400,000.
Life Insurance for Your Dependents
In order to elect life insurance for your dependents, you must
elect life insurance for yourself.
OhioHealth provides you with the opportunity to choose one of
two dependent Life Plans. This insurance is available through
UNUM Provident Life Insurance Company. Options are as follows:
Basic Dependent Life
Spouse
$10,000
Child 6 months to 21 years (25 if full-time student)
$5,000
Infant live birth but less than 6 months
$1,000
Optional Dependent Life
This plan provides you with the opportunity to elect the coverage
amount for your spouse and/or dependent children. Options for
spouse is $5,000 increments up to $100,00 (EOI required for amount
above $25,000 and for future increases). Options for child(ren)
are $5,000 or $10,000.
Accidental Death and Dismemberment (AD&D) Insurance for
You and Your Family
Accidental Death and Dismemberment Insurance helps protect against
financial loss if you or a family member is severely injured
or killed in an accident. The amount of benefit that AD&D
pays depends on the coverage you select. AD&D is strictly
for injuries or death as a result of an accident. You can choose
from the following benefit options for yourself, or yourself
and your family: $25,000; $50,000; $100,000; $150,000; $200,000;
$250,000.
Long-Term Disability Insurance (LTD)
Long-Term Disability Insurance protects your income if an injury
or illness keeps you from working for an extended period of time.
This program picks up where the Temporary Disability Pay (TDP)
program leaves off. LTD coverage replaces 60% of your pre-disability
pay and begins paying benefits when you have been unable to work
due to an injury or illness for more than 120 days.
Business Travel Accident Insurance
OhioHealth provides $50,000 of travel accident insurance.
Long-Term Care
OhioHealth offers Long-Term Care coverage through AFLAC, which
provides the assistance you need if you or your spouse has a
chronic condition leaving you unable to care for yourself for
an extended period of time. Benefits include:
- Nursing home care
- Assisted living
- Home health care
- First occurrence benefit when diagnosed as being critically
ill
Accident Protection Policy
OhioHealth offers protection to you through AFLAC in the event
of an accident. This Personal Accident Expense Plan is designed
to help cover the expenses associated with an accidental injury.
It pays you directly, unless you assign benefits, regardless
of any other insurance you may have. Benefits include:
- Accident hospital confinement
- Accidental death and dismemberment
- Accident emergency treatment
- Transportation, family lodging
- Wellness benefits
Auto and Home, Pet and Legal Insurance
OhioHealth has teamed with MetLife to offer the following programs.
You can pay for them through convenient payroll deductions.
Auto & Home Insurance
This program features benefits and conveniences that are designed
to save you money. Through this program you can apply to insure
your auto, home, other property and yourself against personal
liability at special group rates.
Pet Insurance
Veterinary Pet Insurance (VPI) can help you handle the every-increasing
cost of caring for your pets when they are ill or injured.
MetLaw
MetLaw provides you and your family with legal services from
experienced attorneys through Hyatt Legal Plans. With MetLaw
you can receive legal advice services for a wide range of legal
matters.
